By bringing in your new patient forms filled out, you will save time on your first
visit. This will allow the doctor to spend more time with you to evaluate and
discuss your condition.
If you would rather the forms be emailed to you, please feel free to call our office
and request any team member to email the forms to you directly.

What to Bring to the First Visit
To help us provide you with the best possible care during your first visit, please bring the below items. Having these items ready will help us streamline your visit and address your needs more effectively.
- Health Insurance Card/Information
- Photo Identification (drivers license or passport)
- A copy of your MRI, X-rays or CT scan reports
- List of of current medications
If you are being referred by a physician please make sure that we have received records and diagnostic
testing. If you are a self-referring patient please make sure you bring your records and diagnostic results
with you to your consultation (if applicable).
